PRE-ADOPTION FORM

1) Do you have any other pets? If yes what are the sexes, ages and breeds of these animals.

2) Do you still have them? If no then what happened to them?

3) Have you ever surrendered a pet to a kill animal shelter? If yes why?

4) Do you own or rent your living quarters? If you rent are you allowed pets?

5) Do you have a fenced in area for the animal to get exercise? If yes, how large of an area?

6) Do you have children? If yes how many and their ages.

7) Have you researched or know about the breed of animal you are adopting?

8) None of the animals we have are perfect they have come to us for various reasons. They may have problems or their owners no longer wanted them. They all will require some time and patience and a lot of love.

9) We have no papers with any of our animals and cannot guarantee that they are purebred. We can pass onto you, what we were told at the time the animal came into our care.

10) We require the address and phone number of your current veterinarian. Be sure to inform them we will be asking for information. Please include it on this application to help make our decision about the adoption.

11) Every animal has to be spayed/neutered when it is of age. A copy of the receipt needs to be sent back to us showing that it was completed. If this is not done, we will call and the animal will be picked up, due to failure to keep up the terms of your adoption agreement. No money will be refunded.

12) Are you employed? If so, how many hours are you working? Where will the animal be kept while you are away?

13) Can you comfortably support a pet? They can sometimes be more expensive than we plan, such as grooming or healthcare.

14) Do you plan to crate train and housetrain?

15) KRS reserves the right to refuse adoption of any pet to an individual, if we feel it is not in
the best interest of the animal.

16) Adoption fees are based mainly upon the animal. We are a no kill shelter, so we try to recover some of the funding we put into each animal.

17) We assume no responsibility for the temperament of the animals. We can only tell you how they act while in our care.

18) We do not guarantee any of our animals against any diseases that can be transmitted from
other animals. We can only tell you how they are while in our care. During the time they are kept at KRS, they are vaccinated and wormed. If sick or appear sick, the puppies or kittens are seen by our local veterinarian. However, some conditions are a little harder to detect.

19) KRS will not hold an animal for any individual unless a deposit is placed to hold the animal. Adoption pending means - the pre-adoption papers have been sent to us and we are making a decision. Once a decision is made, the adopter has 2 days to make arrangements to pick up the animal. If for some reason, they do not complete this in the time required and they haven’t made new plans with us, then the pet will be passed down to the next approved adopter.

20) To adopt an animal from us, you must answer all the questions completely and read this adoption form as well as date it. Once it is received, we will review it and make a decision. We will contact you and arrange a date to meet and finish signing adoption papers and add a new addition to your home.
